You can get up to $14 back on any Delsym purchase made between November 1, 2014 and March 31, 2015 once you submit for your rebate.

Limit one per household or mailing address, and you must submit your original receipt.  Also, note that the rebate form is a PDF file, which means you might not be able to open it on some phones.
